For new patients that are on short-term or long-term disability, we must inform you that we cannot guarantee continuation of your disability benefits. New patients must be in treatment and care with our medical practice for at least three continuous months. We must monitor the patient's response to treatment and medication regimen before making a determination if medical criteria is met and disability benefits can be granted.
